Medication permission form – please bring with the medication and drop both off at the nurse’s office on the first day of camp or if your child begins new medication
Day Campers Ages 3 and 4
- Department of Health forms – provided and completed by your doctor
- Parent preschool form – completed by parent
- Please read “Know your Child Care Center”
Day Campers Ages 5 to 14
- Day camp medical form – indicating immunization dates
Kampus Kampers
- Overnight camp medical form – must be completed by your doctor
- Confidential questionnaire – completed by parent
